Custom Development
Build the thing that does not exist yet.
Architecture, engineering, and analytics as one engagement, because splitting them across three vendors is how the seams become somebody else's problem.
Custom Development covers the three things a quality organization usually needs built rather than bought: the governed data foundation underneath the measures, the integrations and automation that move data between systems that were never designed to talk, and the value based care analytics layer that turns the result into something a person can act on. Scoped and staged, on a documented stack, with the runbook written as we go rather than promised at the end.
The problem
Custom work has a reputation problem it earned. It runs long, the scope moves, and it ends with a system only the builder understands. The failure is almost never technical. It is that nothing was staged, so there was no point at which the work was demonstrably half done, and nothing was documented, so the knowledge left when the contract did.
What we build
Governed data architecture
warehouses, pipelines, and lineage designed for audit from the start, not retrofitted once somebody asks where a number came from.
Integrations
connections between systems that were never designed to talk, with failure handling and a written contract for each interface.
Workflow automation
the manual steps between a data arrival and a decision, removed and instrumented so you can see when they stop working.
Analytics and reporting
the measure views, cut point exposure, and operating reporting that sit on the foundation, with definitions written down.
Bespoke applications
internal tools where a workflow genuinely does not exist off the shelf, built on a stack your team can hire for.
FHIR ready foundations
structured for the move to digital quality measurement rather than against it, so the next specification change is a version bump.
What it looks like in practice
A plan has claims in one system, supplemental clinical data in another, abstraction tracked in a spreadsheet, and a vendor rate file arriving monthly by email. We build the governed foundation that consolidates them, the automation that keeps them current, and the reporting layer where the quality lead and the analytics lead finally see the same number.
Staged, so it is never all or nothing
Every engagement is broken into stages that each produce something usable. If the work stops after stage two, you keep what stage two built and it functions on its own. That is a constraint on how we design, not a billing convenience, and it is the main reason custom work fails when it fails.
Documented well enough to leave
Runbooks, architecture decision records, and a named internal owner on your side from the first stage. The test we hold ourselves to is whether your team could operate it if we disappeared. Work that only we can maintain is work we have done badly, and it is also how a vendor quietly becomes unremovable.
Your stack, in your name
Your cloud tenant, your repositories, your licenses, all in your name with a documented exit process. We do not hold infrastructure or credentials as leverage, and we will say at the outset which dependencies you are taking on.
Who it is for
Stop rebuilding the same model every year. Let AI handle the routine ad-hoc requests so your team can build the predictive future instead of being the report desk, on a maintained registry with versioned specifications you can inspect. You are the person a security review runs through, so the first product asks for nothing that needs one.
You know what the strategy should be. You are waiting on a ticket for the numbers, and the two things you fear most are a half star drop and a failed audit. Walk into the executive suite with a ranked, dollar weighted plan at the start of the measurement year, instead of a year end surprise.
If the foundation is the problem
When the constraint is provenance rather than tooling, Provenance First Data Architecture is the engagement that addresses it directly.
